Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you need to complete our travel vaccination questionnaire. The best way to complete this is online by registering for our online services, then going to the 'Questionnaires' section and submitting the 'Travel Questionnaire'

It is important to complete this questionnaire as early as possible - at least 6 weeks before you travel. Some vaccines may have to be ordered in specially. 

Please contact the surgery (via online message or call after 10am) a week after completing the form. This will give the nurse time to look through your record and make the correct recommendations. You can then arrange an appointment with a nurse to have the vaccines administered. This appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.  

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS
