Patient Led Support Groups

Practice Health Champions

We are lucky to have a dedicated group of patients who have formed the 'Practice Health Champions '. With the support of the practice, they are giving up their time to volunteer and help other patients with initiatives such as waiting room support, and self help groups.

Leylands Bereavement Support Group

The Leylands Practice Health Champions have set up a Bereavement Support Group, meeting every two weeks on Friday afternoon at the Leylands surgery. The aim is to provide support to patients who have been experiencing difficulties following a bereavement. The health champions will provide that sympathetic ear & help patients take appropriate actions to address any administrative issues related to the bereavement, but more especially to ‘re-balance’ their lives & social networks.

The health champions will assist & support the patient in making contact with the relevant organisations professionally trained to deal with any issues highlighted.

This is a service that has been setup by patients for patients. Please enquire at reception if you wish to attend, or indeed if you wish to volunteer your time to help.

Other Bereavement Support Groups
